This is in response to the letter by Dr. Pudukadan related to the manuscript entitled, “Role of real-time intraoperative optical coherence tomography (OCT) in lamellar corneal surgeries.”

Thanks for the interest shown in the manuscript.

The intraoperative OCT can display real-time clear images of corneal layers such as epithelial, stromal, and descemet's layers. The resolution of up to 10 μm is possible, so even descemet's and endothelial layers can be made out during the lamellar corneal procedure. While performing the dissection, the exact depth at which the dissection is being done cannot be seen because the dissecting instruments (e.g., crescent blade or lamellar dissector) can cause shadowing of corneal structure directly under the instrument; however, by taking out the dissecting instruments and lifting the dissected layer, the remaining layer and its thickness can be made out and this helps the surgeon to know the depth of dissection. It not only allows a safe dissection but also assists the surgeon to keep a uniform residual stroma for a good interface in anterior lamellar keratoplasties.

It is not possible to integrate the normal OCT machine in microscope as it is in-built into the microscope without being mounted on separate arms. The RESCAN 700 is a specially designed OCT system that has been integrated to LUMERA 700 for providing intraoperative images.
